On the practical, ethical, and legal necessity of clinical Artificial Intelligence explainability: an examination of key arguments
This systematized review critically examines arguments for and against the necessity of explainability in clinical Artificial Intelligence (cAI). We find that while autonomous 'black box' algorithms are problematic, cAI without explainability can be integrated into human-in-the-loop decision-making, aligning with existing evidence-based medicine practices. Novel comparisons to idiopathic diagnoses and diagnoses by exclusion, alongside an analysis of clinical practice guidelines, suggest that a lack of cAI explainability does not inherently compromise clinician due diligence or epistemological responsibility. Many arguments for explainability conflate unexplainable AI with automated decision-making. We conclude that current literature does not universally necessitate cAI explainability, particularly in human-in-the-loop contexts.
